Make A Claim For Workers’ Compensation Benefits Within Two Years

This step is necessary to receive coverage for medical treatment and lost wages. Under Michigan law, a claim must be made within two years of the workplace injury. What If My Employer Will Not Cooperate?

This form will start the workers’ compensation process, regardless of the involvement of your employer.
